How to use this template

  1. Confirm the submission level — Level 3 requires all 18 elements submitted to the customer. Confirm with your customer SQE which elements are required vs. retained at the supplier.
  2. Download and print the checklist — click the button above. Use this as your master tracking sheet throughout the PPAP preparation process.
  3. Work through each of the 18 elements — mark Required (Y/N), record the current Status (Complete / In Progress / N/A), and note the document title or reference in the Notes column.
  4. Complete Element 18 (PSW) — the Part Submission Warrant is the cover sheet for the entire package. It must reference the drawing revision, PPAP level, and submission reason. Obtain engineering and quality signatures.
  5. Assemble the submission package — organize all required element documents in element number order. Attach this checklist as the index page of the submission.
  6. Submit and track approval — submit to the customer SQE. Record the approval date and any interim approval conditions in the Notes column of Element 18.

PPAP Note: A new PPAP is required for any design change, process change, tooling change, or lapse in production of more than 12 months (AIAG PPAP 4th Ed.). Keep the approved PPAP package on file for the life of the part plus any contractually required retention period.
