How to use this template

  1. Determine FAI scope — identify whether this is an initial production FAI, a change-based partial FAI, or a full re-FAI. The scope drives which forms and characteristics must be completed.
  2. Complete Form 1 (Part Accountability) — record the part number, drawing number/revision, order number, FAI type, and organization fields. This is the traceability anchor for the entire FAI package.
  3. Complete Form 2 (Product Accountability) — document all applicable material certifications (raw materials, hardware, consumables), approved special processes, and functional test results per the design documentation.
  4. Complete Form 3 (Characteristic Accountability) — measure and record every characteristic called out on the drawing or specification. Every balloon on the drawing should map to a row in Form 3.
  5. Record nonconformances — if any characteristic is out of tolerance, record it in the Nonconformance column of Form 3 and initiate an NCR or deviation request. Do not pass a FAI with open nonconformances.
  6. Obtain approvals — the FAI package must be signed by the Design Activity representative and the FAI Approval Authority before the part is released to production.
  7. Submit to customer — provide the complete FAI package (Forms 1-3 plus supporting certs, test reports, and inspection data) to the customer or design authority as required by the purchase order or contract.

AS9100 / AS9102 Note: A FAI is required by AS9100 Rev D Clause 8.1.3 for each new part and whenever a process, material, or design change affects form, fit, or function. Retain the complete FAI package for the life of the part plus a minimum of 10 years, or per customer requirement.
