How to use this template

  1. Define the background — explain the business context, strategic relevance, and why solving this problem is important now.
  2. Map the current condition — describe the current state with specific data: defect rates, cycle times, process maps, or customer feedback.
  3. Set a measurable goal/target — write a SMART target: specific metric, target value, and deadline (e.g., reduce defect rate from 4.2% to <1.0% by end of Q3).
  4. Analyze root causes — use 5-Why or fishbone analysis. Focus on systemic causes, not just symptoms. Confirm the root cause with data.
  5. Develop countermeasures — list concrete actions that directly address the root cause. Each countermeasure should map to a specific cause.
  6. Build an implementation plan — assign an owner, action, and due date for every countermeasure. Track as a simple table.
  7. Follow up and confirm — after implementation, measure results and compare against the target. Document what worked and capture lessons learned.

A3 philosophy: The A3 process is a thinking tool, not just a form. The discipline of fitting everything on one page forces you to prioritize what matters most. Work through it collaboratively with the team closest to the problem.
