How to use this template

  1. Schedule the audit walk-through — identify the area to be audited, assign an auditor independent from the area, and set a date.
  2. Download the checklist PDF — generate and save the template to your device.
  3. Complete the audit header — fill in the area name, audit date, and auditor name before walking the floor.
  4. Score each S category — walk the area and evaluate all five items under Sort, Set in Order, Shine, Standardize, and Sustain using the 0–5 scale.
  5. Tally category and total scores — sum the five item scores for each S (max 25 per S) and add all five S totals for the overall score (max 125).
  6. Log action items — for any item scoring below 3, describe the finding, assign a responsible person, and set a target date in the action items table.
  7. Sign off and follow up — obtain auditor and area supervisor signatures. Review open actions at the next audit cycle.

Scoring guide: 0 = No evidence of the practice; 1 = Initial awareness; 2 = Partially implemented; 3 = Implemented but not sustained; 4 = Well implemented and mostly sustained; 5 = Fully implemented, sustained, and continuously improved.
