Document Change Request Form

A Word form for proposing a change to a controlled procedure, work instruction or form. It records what changes and why, what else the change affects, who approved it and how it was rolled out.

Who this template is for

Anyone who finds a procedure, work instruction or form that is wrong or out of date, and the document controller, process owner and quality approver who process the request.

When to use it

Standards it supports

How to use this template

  1. Describe the request — enter the document number, title and current revision, and tick the type of change.
  2. Show the change — for each section, write the current content, the proposed content and the reason. Attach a marked-up copy for long changes.
  3. Assess the impact — list other documents, forms, records, training, equipment and customer approvals the change affects.
  4. Review and approve — route the form to the process owner and quality, who approve, reject or send it back for revision.
  5. Implement and close — issue the new revision, update the master list, withdraw old copies, train the people affected and close the request.

Tip: Write the reason as the problem, not the solution: "Operators skipped the coolant check because it was not in the steps" tells reviewers more than "add coolant check".

Frequently asked questions

What is the difference between a DCR and an ECN?

A document change request covers procedures, work instructions and forms. An engineering change notice covers product design and drawings, where fit, form, function and customer approval matter.

Who should approve a document change?

At least the process owner and the function responsible for the QMS, usually quality. Add others when the impact assessment shows a change affects them.

Does a typo need a full DCR?

Many companies allow an administrative correction path with a lighter review, as long as no requirement changes. Define the rule in your document control procedure.

How long should a DCR take?

Set a target in your procedure and track it. Long approval times push people to work around the documents.
